Intracranial aneurysms embolization with coils, without extra devices – the single – centre experience

Abstract
<b>Introduction and aim. </b>We conducted a retrospective analysis of endovascular treatment of intracranial aneurysms without any additional equipment, both ruptured and found accidentally. To assess the results of embolization of aneurysms, regardless of the size of the bag and the width of the neck, using only the coils.<br><b>Material and methods. </b>The study included 60 patients with 67 aneurysms treated by embolisation from 06.2010 to 06.2011. Sizes of aneurysms were evaluated using angiography with 3D reconstruction. The study assessed: efficacy of treatment, the incidence of complications, the result of angiography at 6 months and clinical outcome.<br><b>Results. </b>Complete closure of the aneurysm immediately after the procedure occurred in 45 (67%) cases, partial or incomplete closure occurred in 22 (33%) cases. During treatment there was no rupture of any aneurysm. In 5 (7%) cases minor complications occurred. In any case, there was no permanent neurological deficits. In 60 patients with 67 aneurysms treated with endovascular embolisation without any additional equipment in the form of balloons, stents or flow-diverters obtained satisfactory results were confirmed by angiography at 6 months, as well as the clinical examination. Embolization was characterized by a low rate of complications and no permanent neurological deficits. For most anterior circulation aneurysms, it is suggested to carry out classical embolization using coils (without the aid of flow-diverters, stents or balloons), which is often more effective and at the same time with the lower risk.<br><b>Conclusions</b>. The embolization procedure is currently the main method of treatment of intracranial aneurysms and is used around the world. In the face of emerging technologies and new devices (stents, flow-diverters, balloons) that are designed to increase the effectiveness of treatment especially aneurysms with a wide neck, detachable spirals remain primary, technically sophisticated embolic material. Many aneurysms, despite the unfavorable features such as a wide neck (neck/sack = <1) can be treated using only the coils. Our group of patients shows that with proper patient selection, use of appropriate technique and operational strategy, treatment is highly effective (88% efficiency, Raymond 1 + 2) safe and does not require the patient’s dependence on anticoagulants.
