Outcome of surgical management of blunt trauma abdomen

Abstract
Background:Blunt abdominal trauma is a condition in which high index of suspicion is required along with investigations to diagnose intraperitoneal injuries. The primary goal in treatment of severe abdominal injury is to save life.Aim: To evaluate the outcome of surgical management of blunt abdominal trauma.Material and Methods:A total of 60 patients with suspected blunt abdominal injury were investigated and managed surgically. Liver tear suturing for simple liver tears and liver pack for uncontrolled bleed from scattered liver was done. Splenectomy done for splenic injuries. Large intestine tear was sutured and proximal colostomy was done.Bladder injuries were sutured and in case of urethral injuries, suprapubic cystostomy was done.Results:Amongst 60 study subjects 25 (41.67%) required procedure. Most common procedure done was primary close (44%) followed by liver packing (32%). About 20% suffered complications among which chest infection was the most common complication followed by bile leak, death and wound infection. We found mortality rate of 1.67% in our sample.Conclusion:A possibility of blunt abdominal trauma should be kept in mind in all cases of polytrauma and vehicular accidents, even when local signs are absent. This is important since a delay in diagnosis and operative intervention results in increased mortality and morbidity
