QEEG and Monitoring of Anticonvulsive Treatment in Epileptic Children

Abstract
Background: The study aimed to reveal EEG baseline patter for optimization of anti-epileptic treatment in individual epileptic patients. Such type of investigation is very important, because characteristics of baseline EEG abnormalities may sometimes signal clinical signs of aggravation. The data on quantitative characteristics of baseline activity of interictal EEG are less. Methods: 35 patients aged 9-13 with different type of epilepsy were investigated. EEGs were recorded three times: prior to Valproic Acid- monotherapy, at three and forth and eight months under therapy. Baseline EEG was studied for quantitative mejerment of EEG such as absolute values of the power (AVP) spectra and EEG topography/Brain Mapping. Results: Valproic Acid- reduce Delta frequency, but did not effect on monomorphic theta-waves of temporal region. The described AVP dynamics was analogous in both hemispheres. No interhemispheric specificity was seen in the dynamics of the frequency ranges analyzed observed. Study of basic characteristics of EEG during the Valproic Acid- reveal that the presence of monomorphic theta-waves may suggest a possible recurrence of seizures after withdrawal Valproic Acid. Conclusion. Spectral analysis and Brain mapping reveal the predicting value of baseline EEG. The presence of monomorphic theta-waves with parietal zone can anticipate reocurrence of seizures if Valproic Acid will be reduced or withdrawn. AED-therapy can be revealed via reduction of low-frequency waves and suppression of epileptiform EEG elements parallel to clinical improvement. Thus, optimal treatment strategies can be tailored based on the evaluation of background EEG characteristics using spectral analysis, EEG mapping and the quantitative EEG approach.
