Role of High Performance Liquid Chromatography (HPLC) screening test in Young population to prevent thalassemia and other hemoglobinopathies.

Abstract

Thalassemia is a heterogeneous group of inherited disorders of hemoglobin characterized by reduced or absent production of globin chains.It is the commonest single gene disorder in india.Inherited disorders of Hb due to the structural alteration of globin chain are called hemoglobinopathies. Material and method : We had screened total 1071 healthy students.The tests were performed on an instrument manufactured by BIO RAD laboratories,USA.The instrument, known as BIO RAD 'VARIANT' (beta thalassemia short program) utilizes the principle of high performance liquid chromatography (HPLC) for various hemoglobinopathies and variants as described in instruction manual forthe assay. Result : Out of total 1071 apparently healthy students screened, we had detected 50 cases which displayed abnormal hemoglobin fractions on HPLC.A total of 25cases (2.33%) of beta thalassemia trait were diagnosed followed by 20cases (1.86%) of sickle cell trait and 5cases (0.46%) of HbD Punjab trait were diagnosed. Conclusion : HPLC has been shown to be as an emerging tool which is sensitive, specific and reproducible alternative to electrophoresis. With automation and quantitative power, it appears to be a sensitive and accurate technique with a high degree of precision in the quantification of major and minor,normal and abnormal, hemoglobin variants.
