Study of reliability of various USG criteria of lymnodes in oral malignancy to detect metastasis preoperatively

Abstract
Background: There are several methods available nowadays to decide and to differentiate the metastatic nodes along with clinical examination like ultrasound examination, USG-FNAC and computed tomography. Out of these, USG is easily available, inexpensive and bed side modalities along with clinical examination. Aim: To know the reliability of various USG criteria in oral squamous cell carcinoma. Material and Methods: A total of50 patients of diagnosed squamous cell carcinoma of oral cavity were studied. The cervical nodes were evaluated by clinical examination, USG and USG FNAC. Then FNAC was done under USG guidance. After surgery neck nodes were dissected and sent for histopathology. From this sensitivity, specificity, accuracy, positive and negative predictive values were derived and compared with histopathology. Results: Thehilum was having highest sensitivity (90%)and lowest specificity(22.72%). Central necrosis was having highest specificity (96.87%) and lowest sensitivity (22.22%). Highest accuracy with shape and RI (72%) was found and lowest with hilum (43.75%). Conclusion: The roundness index, shape and central necrosis are reliable criterion to differentiate malignant and benign nodes because of their high specificity and accuracy.
