SURGICAL RETRIEVAL OF A FORGOTTEN COMMON BILE DUCT STENT FOLLOWING PENETRATION THROUGH THE THIRD PART OF DUODENUM –A RARE CASE REPORT

Journal Title: National Journal of Medical and Dental Research - Year 2018, Vol 6, Issue 2

Abstract

Background:Endoscopic biliary stenting is a well-established and minimally invasive diagnostic and therapeutic modality for obstructive hepatobiliary and pancreatic diseases. Biliary Endoprostheses migrationoccurs in up to 10% of patients undergoing stentingand about 1% of migrated stents are associated with grave complications like gut penetration or perforation. Case study: We report aninteresting case, where a common bile duct stent, forgotten for about 5years,migrated distally and penetrated the third part of the duodenum. The stenthad to be retrievedsurgically. Conclusion:Forgotten endobiliary prostheses can migrate and cause gut perforation and other life-threatening complications. Hence, stented patients need regular post procedure follow up and continuous surveillance until the timely removalor changes of the biliary stents.Endoscopy units must mandatorily maintain an endobiliary prostheses registry system.Although endoscopy is treatment of choice to retrieve migrated stents, surgical retrieval is needed whenever gut perforation is suspected.
