The pancreatico-pleural fistula - About one case

Abstract

Pancreatic pleurisy is rarely encountered in the daily practice of pneumology. It is a rare pathology that often remains under diagnosed. We report the case of a 58-year-old patient, with a notion of passive smoking, having an medical history of hospitalization for acute pancreatitis, admitted for the management of a pleural effusion on the left with great abundance. A pleural puncture made showed a pleural fluid of cloudy and exudative appearance, with a pleural lipase at 13,773 IU/L. The biological assessment shows a microcytic hypochromic anemia at 10.5 g/dL and the blood lipase at 188 IU/L. Hepatic and renal function are normal, the phtysiological assessment is negative, A thoraco-abdominal CT scan shows a left pleural effusion associated with two false communicating cysts of the pancreas causing suspicion of a pancreatic-pleural fistula despite the non-visualization of fistulous tract. In front of the appearance of pleural fluid, a thoracic drainage had been instituted, combined with a medical treatment based on antibiotherapy and a physiotherapy allowing a favorable evolution of the disease with total regression of the pleurisy.
