The role of Cytokeratins as additional marker in breast carcinoma

Abstract

Background:In recent years, breast carcinomas have been categorized into subtypes depending on their gene signatures. Using immunohistochemistry (IHC) it is possible to assign tumors to various molecular subtypes which can be a useful surrogate of gene expression analysis and has a wider application. Cytokeratins (CK) are used as differentiation markers in breast cancer since their expression remains stable during carcinogenesis. In breast ducts CK 7and 18 are expressed in the luminal layer, whereas CK 5/6 and 14 characterize the basal layer.Objectives: To determine the frequency of expression of each cytokeratin subtype and its correlation with age, histologic grade, lymph node status andestrogen (ER),progesterone(PR) and Her-2/neu receptor status in breast carcinoma.Materials and Methods:A 12 month prospective study was conducted on 55 cases of modified radical mastectomy specimens in the department of pathology of our institute. All the cases were subjected to routine histopathology and IHC.In each case all relevant clinicopathological data were recorded andanalyzed.Results: We were able to subtype the cases into four distinct phenotypic categories- pure luminal 25(45.46%), pure basal 2 (3.64%), basoluminal 25(45.46%) and null 3(5.45%). There was a significant correlation between histologic grade and ER/PR/Her-2/neu expression among four subtypes. A significant association was found between higher histologic grade, ER/PR negativity, Her-2/neu overexpression and basal/basoluminal phenotype.Conclusion:Addition of cytokeratins to the battery of IHC markers has helped to categorize into distinct subtypes, requiring different therapeutic approaches for improving disease free survival
