Tranexamic acid in intensive therapy of bleeding with different genesis

Abstract

The problem of acute bleeding is relevant both in the medical and in the organizational aspects, and not only for Ukraine. The success of treatment is determined by a timely diagnosis and the speed of emergency medical care. Emergency surgical hemostasis, the use of antifibrinolytics, and the restoration of hemodynamics and blood loss contribute to arrest of hemorrhage. Tranexamic acid is listed as an essential drug. This drug has an antifibrinolytic effect, enhances collagen synthesis which helps preserve the fibrin matrix, stabilizes the thrombus, normalizes platelet function and capillary permeability, and prevents the formation of bradykinin, kallikrein, T‑kinin. The experience of using tranexamic acid in injured individuals with signs of massive bleeding, in extensive surgical interventions, in patients with malignant neoplasms and postpartum hemorrhage is highlighted. Clinical studies have shown the efficacy of tranexamic acid in combat injuries, preparing patients for coronary artery bypass grafting, total hip and knee arthroplasty, major surgical operations on the spine, uterine, nasal, gastric and intestinal bleeding, hematuria, bleeding after prostatectomy and tonsillectomy. It has been proven that tranexamic acid reduces the mortality of patients from bleeding during childbirth, in the perioperative period and during injuries, without increasing the risk of thrombotic complications. The approaches are suggested to the use of tranexamic acid for bleeding of various origins according to the British recommendations, the guidelines of the European Association of Anesthetists for the intensive care of massive bleeding, recommendations for tactical medicine of the United States and the American Society of Anesthesiologists. The problem of the effectiveness of the use of this drug is highlighted.
