Role of liposuction combined with webster’s procedure in the surgical treatment of Gynecomastia

Abstract
Background: Gynecomastia is swelling of the breast tissue in males, caused by an imbalance of the hormones oestrogen and testosterone. Gynecomastia can affect one or both breasts, sometimes unevenly. This condition differs from pseudo gynecomastia (fatty breasts), which usually occurs in obese men, due to increased local fat deposition without glandular enlargement. Aims and Objectives: To study the effectiveness of Liposuction combined with Webster’s procedure in the treatment of Gynaecomastia. Materials and Methods: In this retrospective study, a total of 10 patients were included with patients presenting to the general surgery OPD and Plastic surgery OPD in the year 2016-2017 in our institute. After taking a thorough history regarding the aetiology, age of onset, laterality, and nipple discharge; clinical examination and relevant investigations were done. Those patients with simon’s grade 3 and with secondary causes of gynaecomastia were excluded. Those with primary gynaecomastia and those willing for surgery were included in the study. Out of 10 patients , 6 had bilateral and 4 had unilateral gynaecomastia. Preoperative marking of the enlarged breasts was done . All surgeries were performed under general anaesthesia. Liposuction was carried out through a small incision along the inframammary crease in the anterior axillary line. With an infra areolar skin incision, glandular tissue excision is done. Intraoperatively breasts are examined for symmetry. Results: 1. In all 10, the indication of surgery was cosmetic. Mean age of the patient was 27 3. Postoperative complications like infection and seroma formation were minimal and were managed conservatively. 4. Out of 10, 2 patients had simon’s grade 2b , but 6 months post op, on follow up, they did not need a skin reduction surgery Conclusion: Liposuction combined with webster’s procedure is a reliable procedure for the treatment of gynaecomastia and offers good chest contour and symmetry.
